Facility Evaluation Report
On 10/29/2025 at 11:30AM, Licensing Program Analysts (LPAs) Andrew Christy and David Doidge arrived unannounced to conduct a Required - 1 Year inspection. LPAs met with staff, Julie Sapitola, and explained the nature of the visit. Facility currently houses four (4) clients out of a max capacity of six (6) clients.
LPAs toured the facility including but not limited to bedrooms, bathrooms, dining area, kitchen, garage, and outdoor area. No bodies of water were observed. A comfortable temperature indoors is maintained at 75.0 degrees Fahrenheit. Smoke and carbon monoxide detectors were functional. Fire extinguishers were observed to be full and last serviced on 03/18/2025. One week of nonperishable and 2-day of perishable food supplies were available. Hot water temperature was measured at 107.5 degrees Fahrenheit in the hallway bathroom. There were adequate lights in each room. First Aid kit is complete. Indoor and outdoor passageways were free of obstruction.
LPAs reviewed four (4) client files and three (3) staff files starting at 12:45PM. All were found to be complete. LPAs reviewed clients' P & I money with log, and no outstanding errors were discovered. Clients' medication and Medication Administration Record (MAR) reviewed and no outstanding errors were found. Emergency Disaster Plan was last reviewed 08/30/2025. Quarterly emergency drills were last conducted 08/14/2025.
No deficiencies cited during visit. Exit interview conducted and a copy of this report was provided to staff member.
