On 05/07/2026 at 1:00 PM, Licensing Program Analysts (LPA) J. Clancy-Czuleger arrived unannounced to conduct a Case Management. LPA met with Administrator David Clawson .
While LPA J. Clancy-Czuleger conducted a complaint investigation (15-AS-20260506091552 ) on 05/07/2026, LPA observed that resident 2 (R2) physicians report was found in R1 medical file.
The deficiency was observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties.
