Facility Evaluation Report
On 2/26/2026 at 1:00PM, Licensing Program Analysts (LPA) J. Clancy-Czuleger arrived unannounced to conduct a Case Management. LPA met with Joseph Dungo, Community Relations Director . Administrator David Clawson was informed of the visit.
While LPA J. Clancy-Czuleger conducted a complaint investigation 15-AS-20250829120512 on 2/26/2026, LPA observed in the resident record that resident’s admissions agreement do not state if residents are agreeing to shared or private rooms.
The deficiency was observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties.
