Facility Evaluation Report
On 06/04/2026 at 8:30AM, Licensing Program Analyst (LPA) Andrew Christy arrived unannounced to conduct a Case Management visit regarding an incident sent on form SOC341. LPA spoke with Program Director, Shanay McDonald, and explained the nature of the visit. There are currently 24 participants today in the program.
The nature of the incident involved a staff member (S1) allegedly hit a participant (P1) on their hand with a phone on 05/22/2026 while traveling on the group bus from Burger King. While at Burger King on 05/22/2026, P1 ran outside at 11:00AM, being upset they were not able to purchase food. When the group got onto the bus at 1:30PM, S1 used their own phone to allegedly hit P1 on their left hand as a disciplinary measure, stating that P1 was being bad. A picture of a bruise from the alleged incident was taken.
An internal investigation by SVS Concord thus far has not reached any conclusion. LPA then spoke to P1 to confirm details of the incident that were reported. P1 gave a story consistent to the initial report, and mentioned it was the same hand as the picture of the bruise. LPA also interviewed the alleged abuser (S1) and additional staff (S2, S3). In addition, LPA interviewed the house manager (W1) where P1 lives.
No deficiencies cited during visit. LPA may come back at a later point regarding this incident. Exit interview conducted. A copy of this report was made available to the program director.
