On 5/08/2025 at 10:00am, Licensing Program Analyst (LPA) Carol Fowler arrived announced to conduct an inspection of the facility. LPA met with Shavila Wright, Regional Director, and explained the reason for the visit.
LPA toured the outside of the facility and witnessed that the facility is undergoing repairs due to a malfunction in the sprinkler system and is currently not accessible at this time. Therefore the LPA was not able to conduct a full annual inspection with care tool.
No deficiencies were cited during this inspection.
