Facility Evaluation Report
On 01/22/2026 at 12:10 PM, Licensing Program Analysts (LPAs) P.Manalo and A. Christy arrived unannounced to conduct a Case Management inspection of Qcare Residential Facility II. LPAs met with staff, John Ventura, to ensure the facility is in compliance with applicable statues and regulations. Assistant Administrator, Gladys Cuevas Villa, came shortly after.
During the visit, LPAs conducted interviews with residents and staff. Based on interviews with the residents, 4 of 6 residents stated that they are happy and like the food at the facility. Interviews with 2 of 2 staff members are comfortable with the living situation at the facility. 4 of 4 staff stated that there are no issues with receiving their paychecks on time and all stated that there is coverage when staff needs to go on break or on their days off. LPAs observed the California Minimum Wage and Industrial Welfare Commission Wage Orders poster but is not the updated year.
LPAs are requesting for staff to send updated LIC500 by 01/23/2026.
LPAs conducted a tour of the physical plant and observed the following deficiencies:
At 12:30 PM, LPAs observed the hot water temperature measured at 127.6 degrees Fahrenheit.
At 12:37 PM, LPAs observed unlock scissors, mallet, and multi-tool keychain.
