The inspector’s account

On 09/25/2025 at 11:15AM, Licensing Program Analysts (LPAs) Andrew Christy and Grace Luk arrived unannounced to conduct a Proof Of Correction (POC) visit. LPAs met with administrator Joaquin Cunanan and explained the purpose of the visit. The facility currently houses five (5) residents.

The following deficiencies cited during the annual visit on 09/11/2025 were cleared during this visit:

87705(f)(2) - The locking latch on the front door was removed, and the child locks were not present on the side gates of the facility.

87465(h)(2) - There are no loose medications in the fridge, and all medications requiring refrigeration were in a locked box.

87303(a) - The shed in the backyard did not have protruding nails anymore. In addition, the broken screen doors have officially been fixed, junk and debris, including ladders, were properly stored or thrown away, and both back gates open with ease in case of emergency.

The following deficiency cited during that same annual visit has a new POC due date:

1569.695(c) - An extension to 09/30/2025 was granted. The drill forms are created and the drills themselves will be ran before or on POC extension date.

Exit interview conducted and a copy of this report, along with the POC letters, was provided to the administrator.
