Facility Evaluation Report
On 09/11/2025 at 9:15AM, Licensing Program Analysts (LPAs) Andrew Christy and Grace Luk arrived unannounced to conduct the 1 year annual inspection. LPAs met with Caregivers Erasno Pana Jr. and Aiamaureen Gatia and explained the purpose of the visit. Administrator Joaquin Cunanan arrived at approximately 10:30AM. The facility currently houses 5 residents.
LPAs toured facility with including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. All outdoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature is maintained at 70 degrees Fahrenheit. LPAs observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 118.5 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum 7 day supply of nonperishable and 2 day of perishable foods. Sharps and chemicals were locked and inaccessible to residents. First aid kit was found to be complete. Smoke detectors and carbon monoxide detectors were found to be operational, and the last service date for the fire extinguishers is 05/08/2025.
LPAs reviewed five residents files and six staff files. The emergency disaster plan was last reviewed 01/12/2025.
