The inspector’s account

On 10/29/2025 at 8:30AM, Licensing Program Analysts (LPAs) Andrew Christy and David Doidge arrived unannounced to conduct a Required - 1 Year inspection. LPAs met with caregiver Sinforosa Arawiran and explained the purpose of the visit. Administrator Josefina Gardner arrived 9:45AM. The facility is currently at max capacity with six (6) residents.

LPAs toured the facility including but not limited to bedrooms, bathrooms, dining area, kitchen, garage, and outdoor area. No bodies of water were observed. Smoke and carbon monoxide detectors were operational. Fire extinguishers were purchased on 10/29/2025. One week of nonperishable and 2-day of perishable food supplies were available. Indoor temperature is maintained at a comfortable 74.0 degrees Fahrenheit. Hot water temperature was measured at 110.4 degrees Fahrenheit in the hallway bathroom. LPAs observed grab bars and non-skid mat in the bathrooms. There were adequate lights in each room. Resident rooms were observed to be cleaned and fully furnished. First Aid kit is complete.

At 10:00AM, LPAs reviewed five (5) resident records and four (4) staff records, all found to be complete. Emergency disaster plan was last reviewed 07/08/2025. Quarterly emergency drills were last conducted 07/08/2025.

No deficiencies cited during inspection. Exit interview conducted and a copy of this report was provided to the administrator.
