The inspector’s account

On 06/18/2025 at 2:15 PM Licensing Program Analysts (LPAs) L. Alexander and D. Doidge arrived announced to conduct a Post Licensing inspection. LPAs met with with Clinical Director, Louise McClellan and Executive Director, James Mercado . This program serves Adult Residential Facility for Persons with Special Health Care Needs (ARFPSHN).

LPAs toured and inspected the facility with Louise and James that included but not limited to the front entrance, Art's 'n Crafts room, Main Class rooms that included four (4) individual class rooms named by the clients: Dreamhouse, Mansion, Maharlika and The Deck respectively. There are posted QR codes that included a self guided tour for clients that have visual impairments. There was one (1) client at the facility who was later picked up. Clients bring their own food to the day program. The facility has one (1) refill station water for clients in which clients bring their own water bottles. Centrally stored medications were locked in the medication room including refrigerated stored medications. Fire extinguishers were last inspected on 09/30/2024. Two (2) first aid kits with radio and flashlights were observed and located in two (2) locations for emergencies. Carbon monoxide and smoke detectors were interconnected and observed operational during visit.
