The inspector’s account

On 07/08/2025 around 11:00 AM, Licensing Program Analyst (LPA) L. Holmes arrived unannounced to deliver the complaint finding for #15-AS-20250508114135 dated 05/08/25. As a result, LPA conducted a case management and met with Richard Pielstick, Executive Director.

On 05/30/25, LPA and S1 conversed about the facility tour conducted on 05/16/2025, and the power outage that occurred on 04/20/25. R1 stated that he/she and other residents were not informed of what was going on at the facility and/or when the power would be back up and running. On 05/16/25, LPA requested emails and other forms of communications to residents and responsible parties (RP) regarding the power outage. S1 stated that the power has never been off very long and therefore the families were not notified. LPA expressed concern of the approximate time frame of the outage from the early AM hours until that evening should have warranted some form of notification to residents and RP's.

Based on LPA’s interviews and records reviewed, the preponderance of evidence standard has been met; therefore, the above allegation is SUBSTANTIATED . Deficiency is cited from Title 22 California Code of Regulations and listed on LIC9099D. Failure to submit proof of correction by plan of correction due date, and any repeat violations within a 12-month period may result in civil penalties.

Exit interview conducted, appeal rights and a copy of this report provided to Richard Pielstick, Executive Director.
