Facility Evaluation Report

06/30/2025 at 11:00am LPA arrived at the facility and attempted an annual inspection. LPA rang the doorbell with no answer. LPA called the facility number on file that was disconnected and called the mobile number and there was also no answer.

On 7/25/25 at 11:45 am LPA arrived at the facility and for a second attempt an annual inspection. LPA rang the doorbell with no answer. LPA called the facility number on file that was disconnected and called the mobile number and there was also no answer. LPA called the additional number provided via email no answer and voicemail box was full.
