Facility Evaluation Report
On 11/13/2025 at 8:30AM, Licensing Program Analyst (LPA) Andrew Christy conducted an unannounced annual required inspection. LPA met with Administrator Maria Cabral and explained the purpose of the visit. and explained the purpose of the visit. The facility currently houses four (4) residents out of a max capacity of six (6) residents.
LPA toured the facility with Administrator including but not limited to bedrooms, bathrooms, kitchen, common area, garage and backyard. No bodies of water was observed. A comfortable indoor temperature is maintained at 71.0 degrees Fahrenheit. Hot water temperature in the residents' bathroom measured 114.1 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. Residents’ bathrooms are equipped with grab bars. There is a minimum of 7-day supply of non-perishable and 2-day of perishable foods. Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced 09/30/2025.
LPA reviewed four (4) resident files and four (4) staff files, all found to be complete. Emergency disaster plan was last reviewed 10/06/2025. Quarterly emergency drills were last conducted 09/22/2025. A review of resident medications and the Medication Administration Record (MAR) showed no outstanding errors.
No deficiencies cited during visit. Exit interview conducted, and a copy of this report was provided to the administrator.
