Facility Evaluation Report
On 10/07/25 around 09:45AM, Licensing Program Analyst (LPA) Andrew Christy arrived unannounced to conduct a Required 1-year inspection. LPA was greeted by Cecilia Hawkins, administrator, and explained the purpose of the visit. The facility is currently at capacity with six (6) residents
LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. All passageways were free of obstruction; no bodies of water were present. There was a comfortable temperature for clients at 71.0 degrees Fahrenheit. LPA observed adequate lighting and furniture in all rooms for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom measured at 111.1 degrees Fahrenheit. There was a 2-day supply of perishable foods and a 7-day supply of non-perishable foods . Smoke detectors/carbon monoxide unit were in operating condition during visit. Fire extinguisher was last serviced on 03/25/2025. First Aid kit observed to be complete.
At 11:30AM, LPA reviewed 5 resident files and 4 staff files. Emergency Disaster Plan was last reviewed 08/25/2025. Quarterly Emergency Drills were last conducted 09/15/2025. LPA also reviewed client's P&I money with log, and found no outstanding errors.
The following deficiencies were found during inspection:
At 12:00PM during file review, all residents did not have an updated Appraisal Needs And Services Plan
At 11:00AM, a container of bleach was found in an unlocked cabinet under the sink. At 11:20, a closet in the garage was left unlocked and had chemicals such as bleach and Comet.
Exit interview conducted. A copy of this report, along with appeal rights, was provided to the Administrator.
