Facility Evaluation Report

On 10/07/2025 at 12:50 PM, Licensing Program Analyst (LPA) David Doidge unannounced to conduct a 1-Year Annual Required Inspection. LPA met with Caregiver Maricris Floro and explained the purpose of the visit. Administrator Marina Suriao was called by phone and arrived at 1:15 PM.

LPA inspected the kitchen, dining area, living room, bedrooms, bathrooms, front, side and back yards. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. Residents’ bathrooms are equipped with grab bars and non-skid mats. Hallway temperature was maintained at 70 degrees Fahrenheit. The hot water temperature was measured in a shared bathroom at 120 degrees Fahrenheit. There is a minimum of one week supply of nonperishable and 2-day of perishable foods. Centrally stored medications, sharps and toxic are locked and inaccessible to residents in care.

Smoke detectors and carbon monoxide detectors were in operating condition. Fire extinguisher was last serviced on 03/07/2025. Emergency Disaster Plan was last posted on 09/04/2025. Emergency disaster and fire drills are conducted Monthly; last conducted on 09/20/2025. First aid kit was observed to be complete.

LPA reviewed five (5) residents records and four (4) staff records; all were complete.
