The inspector’s account

On 03/21/24 at 09:00 am Licensing Program Analysts (LPA) J. Clancy-Czuleger arrived unannounced to do an annual inspection. LPA meet with Administrators Carmen Palacios and Gay Paris Arndt, and explained the purpose of the visit.

LPA inspected the facility inside out. There is no body of water. Physical plant is consistent with the facility sketch received by Central Application Bureau (CAB) and approved by the fire department. LPA inspected the activity spaces, offices, med rooms, computer rooms, kitchens, bathrooms, and sensory room. There is a comfortable room temperature of 70 degrees Fahrenheit for clients in care. Clients bring their own lunches and snacks but emergency supplies, including water were observed. Centrally stored medications, toxins and sharp objects were locked and inaccessible to participants. Activity supplies were available. Outdoor activity space was observed furnished with tables, chairs and shade. Incontinent clients are kept clean and dry, and the facility is free of odors . The program has 7 vans used for client outings. The facility has a mitigation plan. Fire extinguishers were observed fully charge and tags showed serviced 03/31/2023.

At 09:50 am LPA reviewed 10 residents records. At 10:45 am, LPA reviewed 6 staff records and 6 of 6 were fingerprint cleared and associated to the facility.

No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.
