Facility Evaluation Report
On 9/16/2025 at 9:00 am, Licensing Program Analyst (LPA) J. Clancy-Czuleger arrived unannounced to conduct proof of correction (POC) visit. LPAs met wi th staff Bricia Castro a nd informed them of the reason of the visit.
On 08/27/2025 LPA J. Clancy-Czuleger conducted a case management where the facility was cited for not having an administrator and listed the POC due dates as 09/10/2025.
The following deficiency were not cleared by visit :
1567.50(b)(2) ARFPSHN meets the administration requirements
Civil penalty of $600.00 is assessed for the period of 09/11/2025 to 09/16/2025 for failure to correct deficiency. Facility is subject to ongoing daily civil penalties until deficiencies are corrected.
