Facility Evaluation Report

LPA Carol Fowler arrived on 6/03/2025 at 11:45am, attempted to conduct a Case Management visit regarding an incident that was reported to CCLD on 5/22/2025. There was no one at the facility. LPA called and left a message with the Administrator Jane Uwakwe. Administrator returned LPAs call 6/4/2025.

On 6/04/2025 at 11:45pm, Licensing Program Analyst (LPA) Carol Fowler conducted an announced Case Management visit regarding an incident that was reported to CCLD on 5/22/2025. LPA met with Jane Uwakwe, Administrator and explained the purpose of the visit.

The incident was reported to Elwyn California main office on 4/16/2025. The Administrator reported that a toilet was backed up the office the office contacted Echo Plumbing and Rooter to repair the problem, Echo Plumbing made a second visit to the home in May an at time it was determined there was a leak in the facility and will need to be evacuated due to safety reason. LPA spoke with Christopher Park Regional Director and when asked if the facility had reported the incident to the Department he stated no. All residents have been safely moved to other Elwyn California facilities.

The deficiency is cited per Title 22 California Code of Regulations and listed on LIC809D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties
