Facility Evaluation Report

On 11/7/2025 at 2:15PM, Licensing Program Analysts (LPAs) G. Luk and A. Christy arrived unannounced to conduct a case management visit in regards to clients from sister facility temporary relocated to this facility. LPAs met with Interim Administrator, Desiree Hufalar and explained the purpose of the visit.

During visit, LPAs were informed by Interim Administrator that all four clients and staff from Elwyn California - Maria temporary relocated to this location. Interim Administrator stated that Elwyn California - Maria is updating the flooring throughout the facility, bathrooms, and kitchen. Interim Administrator provided documents that CCLD was notified of this temporary relocation in mid October 2025. LPA obtained a copy of the client's roster. SIRs were sent to CCLD when clients moved to this facility on 11/3/2025.

No deficiencies are being cited on this date.
