Facility Evaluation Report

On 10/01/2025 at 10:00AM, Licensing Program Analysts (LPAs) Andrew Christy and David Doidge arrived unannounced to conduct a Case Management regarding an Unusual Incident report, UIR LIC624 received on 09/29/2025. LPAs met with administrator Corazon Casilihan and explained the purpose of the visit.

Facility is currently at capacity with six (6) residents.

During visit, LPAs collected the following documents: R1's LIC602, R1's Appraisal Needs And Services Report, and R1's Medication List. LPAs interviewed S1, S2, and spoke with S3 via phone call.

LPAs discussed future steps to ensure the safety of the residents with Administrator, including speaking with R1's case manager.

Exit interview conducted and copy of report was provided to administrator.
