Allegation: Facility staff spoke inappropriately towards client

During the investigation the LPA interviewed W1, S1, S2 and S3 during interviews with W1 it was revealed that W 1 received a report alleging that S1 had spoken inappropriately to C1 while see one was residing at the facility. S1 reported that S1 has never spoken inappropriately to C1 or any other clients. S1 further stated that this was an old complaint that had previously been investigated by the clients right advocate for RCEB and was determined to be unsubstantiated. S1 further stated that S1 saw C1 walking down the street and S1 stopped to say hi to C1 and S1 ended up conducting a tour at S1s new day program. S2 reported that S2 never spoke inappropriately to C1 and has never witnessed any staff speaking to C1 in an inappropriate manner during S2 interview, the LPA overheard S1 instructing S2 on how to respond to interview questions as a result the interview with S2 was ended immediately and S2 was determined not to be a credible witness. S3 reported that S3 has not witnessed any staff speaking inappropriately to C1 and stated that S3 was never spoken inappropriately to C1. Therefore, this allegation is UNSUBSTANTIATED.

Allegation: Facility staff are removing personal items without permission from clients

During the investigation the LPA interviewed W1, S1, S2, and S3. During the interviews with W one it was revealed that W 1 was informed that S1 had removed C1's cell phone. S1 stated that S1 never removed or took possession of C1 cell phone. S2 was determined not to be a credible witness as the LPA overheard S1 in the background instructing S2 on how to answer interview questions period S3 stated that C1 had a cell phone and that it remained in C1's possessions at all times. Therefore, this allegation is UNSUBSTANTIATED.
