Facility Evaluation Report

On 8/5/2024 at 12:35pm, Licensing Program Analyst (LPA), L. Hall arrived unannounced to conduct a health and safety check as a result of the department receiving a complaint on 7/31/2024. LPA met with Mary Ann Leyba, Administrator, and explained the reason for the visit.

Upon arrival there were no clients present at the facility.

During the health and safety check, LPA toured the facility including but not limited to common areas, bathrooms, bedrooms and outdoor area. LPA observed one (1) (6) client eating lunch. The facility is noted to be clean, in good repair, and clients in care appear to be safe. There is a minimum of 7-day non-perishables and 2-day perishables foods. There are no imminent health/safety concerns on today's date.
