On 10/16/2025 at 11:05am, Licensing Program Analyst (LPA), L. Hall conducted an unannounced case management visit to obtain facility information for residents. LPA met with Ma Sanelita Rivera, Caregiver, and explained the reason for the visit.
During the visit LPA toured facility and reviewed the six (6) residents files. LPA obtained a copy of the resident roster and LIC500 (personnel report).
Exit interview conducted and a copy of this report provided.
