Facility Evaluation Report
On 07/01/2026 at 3:00PM, Licensing Program Analyst (LPA) Andrew Christy arrived unannounced to conduct a case management visit to deliver an immediate exclusion letter. LPA met with administrator, Habib Jafici, and explained the purpose of the visit.
During visit, LPA hand delivered the immediate exclusion letter for a staff member (S1) to the administrator. LPA was informed that S1 was poised to be hired when given an application 05/01/2026, but there were issues with getting fingerprint clearance. S1 never got these issues fixed. S1 was therefore never officially hired, so the administrator was unaware why S1 was associated to the facility. Administrator will go ahead and disassociate the employee from the facility.
No deficiencies cited during visit. Exit interview conducted and a copy of this report was made available to the administrator.
