Facility Evaluation Report

On 07/10/2025 at 12:15PM, Licensing Program Analysts (LPAs) Andrew Christy and Jill Clancy-Czuleger arrived unannounced to conduct an annual required inspection. LPAs met with administrator Chaba Bender.

LPAs toured the facility including but not limited to the front entrance, kitchen, bathrooms, bedrooms and common areas. Emergency Disaster Plan, Complaint poster, Personal rights, Cough/sneeze etiquette, proper hand-washing signs were observed posted in common areas. Facility has a sufficient 2-day perishable and 7-day non-perishable food supply. Comfortable temperature is maintained at 72 degrees Fahrenheit. Hot water temperature was measured at 113.5 degrees Fahrenheit. Inside and outside pathways were free of obstruction and fire hazards. Smoke and Carbon monoxide detectors were operational. Sharps and chemicals were kept secured and locked from resident access.

LPAs reviewed 5 resident files and 5 staff files, all found to be complete. Appraisals were discussed with the administrator to ensure they are reviewed and signed annually. Facility has emergency disaster plan that was last reviewed 07/19/2024. Facility also conducts fire drills quarterly with the last drill done 04/29/2025. Facility also has up to date liability insurance.

No deficiencies were cited during inspection. A copy of this report was made available to the administrator.
