Facility Evaluation Report

On 4/13/2023, while LPA was conducting tour at the facility due to a different type of visit. LPA conducted an unannounced case management visit.

LPA toured the facility and observed the following:

At around 10:30AM, LPA observed a gallon of bleach at the kitchen counter, which was accessible to residents in care.

Deficiency is cited per Title 22 California Code of Regulations and listed on LIC9099D. Failure to submit proof of correction (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.

Exit interview conducted. Appeal Rights and a copy of this report provided to Administrator.
