Facility Evaluation Report

On 09/11/2024 at 10:40AM, Licensing Program Analyst (LPA) T. Syess-Gibson arrived unannounced to conduct an Annual 1-Year required inspection. LPA met with Christine Schwab, Administrator/ Director, and explained the reason for the visit. The facility's fire clearance was approved for twenty-two (22) ambulatory and two (2) non-ambulatory clients

LPA inspected the facility with , Christine Schwab, Administrator which included but not limited to the bathrooms, kitchen, activity room and common areas of the facility. LPA observed the facility to be free of odor, clean and in good repair. There is a comfortable room temperature of 74 degrees Fahrenheit for clients in care. Clients bring their own lunches and snacks to facility. The hot water temperature in the shared bathroom measured 124.2 degrees Fahrenheit . All observed toilets and hand washing stations are maintained in safe, sanitary and operating condition. There are no bodies of water or fire safety hazards observed. Carbon monoxide and smoke detectors were found to be in working order. No medications are administered at the facility, toxins and sharp objects were locked and inaccessible to clients. Emergency disaster plan last updated 07/07/2025. Fire extinguisher last services on 04/28/2025. Fire drill was last conducted on 07/08/2025. First aid kit was checked and is complete.
