Facility Evaluation Report

On 05/20/2026 at 2:36PM, Licensing Program Analyst (LPA) T. Syess-Gibson arrived to conduct an unannounced 1-Year Required inspection. LPA met with Administrator Charlotte Rile and explained the purpose of the visit. The Administrator currently holds a certificate (#6069669735) that expires on 12/08/2026. The facility’s fire clearance was approved for six (6) ambulatory clients.

LPA toured the facility with Administrator including but not limited to bedrooms, bathrooms, kitchen, common area and back yard. The facility consists of four (4) bedrooms and three (3) bathrooms. All outdoor and indoor passageways are kept free of obstruction. LPA observed a swimming pool behind a gate with a lock. A comfortable temperature is maintained at 71 degrees Fahrenheit. LPA observed lighting in all rooms is adequate for the comfort and safety of the clients. The hot water temperature in the clients’ shared bathroom was measured at 110.5 degrees Fahrenheit. Clients’ bathrooms are equipped with paper towels and hand-washing posters. There is a minimum of 7-day supply of non-perishable and 2-day of perishable foods.

Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last purchased on 05/05/2026. Emergency Disaster Plan was last posted on 04/01/2026. First aid kit was observed to be complete. No fire drill was conducted; facility has no clients at this time.
