Facility Evaluation Report
On 1/21/2026 at 9:40AM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to conduct a Required - 1 Year inspection. LPA met with Program Director, Vy Le and informed her the reason for the visit.
LPA toured the facility including but not limited to activity rooms, resting room, common areas, bathrooms, kitchen, and outdoor area. Smoke and carbon monoxide detectors were observed. Fire extinguishers were observed to be full and last serviced on 9/18/2025. Clients bring lunch from home and facility can provide lunches for clients if needed. Facility has sufficient snacks and food supplies for clients. Hot water temperature was measured at 107.6 degrees F in the kitchen sink. All observed toilets and hand washing stations are maintained in a safe, sanitary, and operating condition. First aid kit was complete. The last fire drill was conducted on 1/9/2026. LPA reviewed 5 clients and 5 staff files starting at 10:25AM. Staff are fingerprint cleared and associated to the facility.
At 11:15AM, LPA observed C4 does not have medical assessment and TB test on file during record review.
The deficiency was observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct deficiency may result in civil penalties.
