Facility Evaluation Report
On 02/24/2026 at 10:50am, Licensing Program Analyst (LPA) L. Hall conducted an unannounced annual required inspection. LPA met with Brandi Pimentel, Direct Care Support. Administrator Christopher Abel, arrived at 11:07am , and LPA explained the purpose of the visit. The administrator currently holds a certificate #7020546735 that expires on 10/01/2027. The facility’s fire clearance was approved for six (6) ambulatory clients. No clients present during inspection.
LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of five (5) bedrooms and two and one-half (2 1/2) bathrooms. Downstairs room used for an office. All indoor passageways are kept free of obstruction. LPA observed a pool with a locked gate. A comfortable temperature for clients is maintained at 70 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 120.4 degrees F. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Paper towel, and soap observed at all hand washing stations. The supply of extra hygiene was available for residents.
Smoke detectors/carbon monoxide were in operating condition during visit. Fire extinguisher was last purchased on 07/07/2025. Emergency disaster plan last reviewed 1/15/2026. Fire drill last conducted 2/1/2026. First aid kit was observed to be complete.
