Facility Evaluation Report
On 07/08/2026 at 9:48AM, Licensing Program Analyst (LPA) T. Syess-Gibson arrived to conduct an unannounced continuation Annual 1-Year required inspection from 07/07/2026. LPA observed no one at the facility upon arrival. LPA called Administrator, Erlinda Portillo, to advise of purpose of visit. Administrator arrived at approximately 10:15AM.
The following deficiencies were observed during visit on 07/07/2026:
At 3:00PM LPA observed unlocked kitchen cabinet with cleaning solution and dishwasher pods.
At 3:03PM, LPA observed a knife inside a kitchen drawer with broken lock.
At 3:20PM, LPA observed S2, S3 and S4 working with clients, S2, S3, and S4 are not associated to facility.
The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiencies by POC date may result in additional Civil Penalties.
