Facility Evaluation Report

On 07/07/2026 at 2:30PM, Licensing Program Analyst (LPA) T. Syess-Gibson conducted an unannounced Annual 1-Year required inspection on this date. LPA met wit Fernando Herrera, Direct Support Staff. Administrator, Erlinda Portillo arrived at approximately 2:45PM. The administrator currently holds a certificate (#7002521785) that expires on 01/09/2028. The facility’s fire clearance was approved for four (4) ambulatory and two (2) non ambulatory clients.

LPA toured the facility with administrator including but not limited to bedrooms, bathrooms, kitchen, common area and back yard. The facility consists of five (5) total bedrooms and two and half (2 1/2) bathrooms. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 77 degrees Fahrenheit. LPA observed lighting in all rooms is adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 101.7 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Hand washing poster, paper towel, and soap observed at all hand-washing stations. The supply of extra hygiene was available for residents. There are a minimum of 7-day non-perishables and 2-day perishables foods. LPA observed five (5) clients and three (3) staff members during visit.

Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced on 11/10/2025. Emergency Disaster Plan was last reviewed on 06/01/2026. First aid kit was observed to be complete.
