On 04/10/2025 at 1:08PM, Licensing Program Analyst (LPA) T. Syess-Gibson arrived unannounced to conduct a case management inspection due to a change of ownership. LPA met with Administrator, Kavitha Pramod.
During Pre-licensing Inspection, LPA observed the following deficiency:
At 12:27PM, LPA observed S5 does not have health screening and TB test on file during record review.
The deficiencies were observed (see LIC 809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties.
