Facility Evaluation Report

On 04/29/2026 at 12:29PM, Licensing Program Analyst (LPA) T. Syess-Gibson arrived unannounced to conduct a case management inspection in regard to incident report received on 04/20/2026. LPA met with House Manager, Lizette Hernandez and Administrator, William Tucker and explained the purpose of the visit.

Incident report dated 04/14/2026 revealed that on 04/14/2026 at approximately 12:45PM client (C1) advised staff of going for a walk, C1 did not return to the facility. C1 returned to the facility on 04/15/2026 at approximately 2:30pm. C1 informed staff of being loss.

Interview with staff revealed that C1 did not exhibit wandering behavior prior and this was the first time C1 wandered from the facility. Interview with staff also revealed C1 moved in on 04/03/2026 and does not have a medical assessment on file.

During record review, LPA observed that C1 has a court order dated 03/24/2026, court order states C1 can not to leave the facility without staff. LPA also observed C1 has a scheduled doctor’s appointment on 05/08/2026. LPA may return at a later date.

No deficiencies were cited during today’s visit. Exit interview conducted and a copy of this report is provided .
