The inspector’s account

On 12/22/2025 at 2:30 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct the Required - 1 Year inspection. The LPA explained the purpose of the visit to Direct Service Provider Marria Clay. Administrator Lizzeth Hernandez arrived at 2:45 PM.

LPA toured the facility, including but not limited to, bedrooms, bathrooms, the kitchen, common areas, the garage, and the back yard. The facility consists of four (4) bedrooms, with one (1) occupied by staff, and three (3) bathrooms. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 71.7 degrees Fahrenheit. The LPA observed lighting in all rooms is adequate for the comfort and safety of the clients. The maximum hot water temperature in the kitchen was measured at 120.0 degrees Fahrenheit. All toilets were in operating condition. The supply of extra hygiene supplies were available for residents.

Smoke / carbon monoxide detectors were in operating condition. Fire extinguisher was full and last serviced on 09/2/2025. The LIC 610D Emergency Disaster Plan had been reviewed 11/1/2025. The first aid kit was observed to be complete.
