Facility Evaluation Report

On 07/15/2025 at 2:11pm, Licensing Program Analyst (LPA) T. Syess-Gibson conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 07/10/2025. LPA met with Joy Bisaha, House Manager, and explained the purpose of the visit.

The incident date of the incident was 07/09/2025. The incident report stated C1 had left the facility and wasn't able to be located. The facility contacted Brentwood Police Department 9-1-1. S1 informed Brentwood Police of C1’s diagnosis and health conditions. The Brentwood Police located C1 at Veterans Park/Senior Center. Brentwood Police interviewed C1, and after interviewing C1 it was determined C1 needed medical attention. Brentwood Police contacted the paramedics who transported C1 to Contra Costa Regional Medical Center at 6:45 PM.

LPA T. Syess-Gibson collected the following documents: C1's physician report, individual program plan (IPP), Brentwood police report number. Joy Bisaha, house manager, will email LPA any similar incident reports and staff schedule for July 2025.

No deficiencies were cited during the visit.
