Facility Evaluation Report

On 04/29/2025 at 3:30PM, Licensing Program Analyst (LPA), T. Syess-Gibson conducted an unannounced case management visit in regard to a substantiated complaint visit on 02/20/2025. LPA met with Joy Bisaha, House Manager, and explained the purpose of the visit. Upon arrival LPA observed two (2) staff members and three (3) clients during visit

LPA T. Syess-Gibson completed an investigation on complaint 15-AS-20250124105320 on 02/20/2025. LPA substantiated the allegation “Administrator is not present in the facility a sufficient amount of time to manage the daily operations” and cited on an LIC9099D. LPA is conducting a case management visit on today’s date to assess a one-time civil penalty of $250.00 for the substantiated allegation 85064(e).

Deficiency is cited per Title 22 California Code of Regulations and listed on LIC9099D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
