Facility Evaluation Report

On 02/13/2025 at 1:15PM, Licensing Program Analyst (LPA) T. Syess-Gibson arrived unannounced to conduct a case management visit. LPA met with Administrator Joy Bisaha and explained the purpose of the visit. Administrator holds certificate #7033899740 expires 10/17/2026. The last visit was on 05/15/2024 because of the Department CCL receiving notification from the Contra Costa Fire Department that the facilities fire clearance has been temporarily suspended.

LPA toured facility with Administrator, Joy Bisaha including but not limited to six (6) bedrooms, three (3) bathrooms, common area, kitchen, and outdoor area. LPA observed no residents at the facility during visit.
