Facility Evaluation Report

On 02/25/2026 at 2:25pm, Licensing Program Analyst (LPA) L. Hall and conducted an unannounced annual required inspection. LPA met Danila Hernandez, Caregiver, and explained the reason for the visit. House Manager, Kimberly Whittle, arrived at 3:15pm. The administrator currently holds a certificate (#7022854740) that expires on 07/15/2026. The facility’s fire clearance was approved for six (6) non-ambulatory residents. Hospice approval for three (3).

LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of six (6) bedrooms and two (2) bathrooms. No bodies of water observed. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. A comfortable temperature is maintained at 73 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 103.3 degrees Fahrenheit.
