Facility Evaluation Report
On 1/14/26 at 12:15 pm Licensing Program Analysts (LPA) J. Clancy-Czuleger arrived unannounced to do an annual inspection. LPA meet with Program Manager Daneila Romero and explained the purpose of the visit.
LPA inspected the facility inside out. There is no body of water. Physical plant is consistent with the facility sketch received by Central Application Bureau (CAB) and approved by the fire department. LPA inspected the activity spaces, offices, quite room, kitchens, and bathrooms. There is a comfortable room temperature of 70 degrees Fahrenheit for clients in care. Centrally stored toxins and sharp objects were locked and inaccessible to participants. Activity supplies were available. The facility has a mitigation plan. Fire extinguishers were observed fully charge and tags showed serviced 01/15/2025.
12:45pm LPA reviewed 5 residents records and 3 staff records and 3 of 3 were fingerprint cleared and associated to the facility.
No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.
