Facility Evaluation Report
On 2/2/2024 at 3:10pm, Licensing Program Analysts (LPAs) L. Hall and T. Syess-Gibson, arrived unannounced to conduct 1-Year Annual Required inspection visit. LPAs met with Executive Director (ED), Scott Shahade, and explained the purpose of the visit. The facility’s fire clearance was 190 non-ambulatory and 10 bedridden residents. Facility has hospice waiver for 18 residents.
LPA requested the following documents to be submitted to CCLD by 2/9/2024.
LIC 308 Designation of Administrative Responsibility
No deficiencies cited during today's visit.
LPAs explained to ED that the inspection will have to be continued at a later date.
