Facility Evaluation Report
On 10/09/2025 at 1:15PM Licensing Program Analyst (LPA) T. Syess-Gibson conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 09/25/2025. LPA met with Leidy Gonzales, Administrator and explained the purpose of the visit.
On 09/25/2025 CCLD received an incident report stating family wants to move C1 out of the facility and S1 is manipulating and mentally abusing C1, scaring him not to stay with family. During visit LPA interviewed S1 and C1 and obtained the following documents: C1’s physician report and admission agreement.
No deficiencies were cited during the visit.
Exit interview conducted and a copy of this report was provided.
