Facility Evaluation Report
On 07/02/2026 at 10:38AM, Licensing Program Analyst (LPA) T. Syess-Gibson conducted an unannounced 1-Year required inspection. LPA met with Nico Jose Dizon, Licensee/Administrator, and explained the purpose of the visit. The facility has an approved fire safety clearance for four (4) ambulatory clients.
LPA toured the facility, including but not limited to, bedrooms, bathrooms, the kitchen, common areas, the garage, and the back yard. The facility consists of four (4) bedrooms, and two (2) bathrooms. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 75 degrees Fahrenheit. The LPA observed lighting in all rooms is adequate for the comfort and safety of the clients. The maximum hot water temperature in client's shared bathroom was measured at 108.1 degrees Fahrenheit. All toilets were in operating condition. The supply of extra hygiene supplies was available for clients.
Smoke / carbon monoxide detectors were in operating condition. Fire extinguisher was full and last serviced on 12/30/2025. The LIC 610D Emergency Disaster Plan had been reviewed on 05/06/2026. The first aid kit was observed to be complete.
