Facility Evaluation Report
On 04/24/2025 at 10:08AM, Licensing Program Analyst (LPA) T. Syess-Gibson conducted an announced pre-licensing inspection. LPA met with Nico Jose Dizon, Licensee/Administrator, and explained the purpose of the visit. The facility has an approved fire safety clearance for four (4) ambulatory clients.
LPA inspected the facility inside and out including but not limited to the bedrooms, bathrooms, common living areas, kitchen, garage, back yard. The facility has four (4) bedrooms and two (2) bathrooms. No bodies of water observed. There is sufficient lighting around the facility. Bathrooms showers/tubs were equipped with non-skid mats. Passageways and hallways are free of obstruction. Locked cabinets available to store medications, toxins and sharps. Hot water temperature is measured at 105.4 degrees Fahrenheit in shared clients' bathroom.
LPA observed facility had some perishable and non-perishable food supply. Licensee will purchase additional food supplies once facility is licensed. First Aid kit was complete. Carbon monoxide and smoke detectors present. Fire extinguisher was last purchased on 12/18/2024. Facility inspection matches the sketch that was provided.
