Facility Evaluation Report

On 11/20/2025 at 2:45 PM , Licensing Program Analyst (LPA) T. Syess-Gibson conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 09/09/2025 . LPA met with John Paul Johnson, Administrator , and explained the purpose of the visit.

LPA received an incident report from Client (C1)’s home stating that C1 passed away while out at a farmers market with C1’s day program, A Step Above .

During the visit, the Administrator clarified that Open Arms is a community based program that shares the same building with A Step Above, and that C1 attended the day program at Open Arms , not A Step Above. LPA informed the Administrator that LPA may return at a later date to gather additional information.

No deficiencies were issued during this visit.

An exit interview was conducted, and a copy of this report was provided to the Administrator
