Facility Evaluation Report

On 4/24/2024, at 10:10am, Licensing Program Analyst (LPA) L. Hall conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 4/21/2024. LPA met with Scott Woods, Direct Care Support and explained the purpose of the visit. LPA spoke with Licensee, Dane McCoy, via telephone.

The incident occurred on 4/19/2025, regarding C1 AWOL'g from facility. S2 stated C1 stated he will be laying down sleeping because he wasn't feel well. S2 went outside to vehicle and observed something on the roof. S2 came back into facility to check on C1. S2 stated he knocked but did not receive a response. S2 then got key to the room, opened door, and observed window open with screen missing. S1 stated facility has a protocol for C1 to leave facility. S1 stated C1 is not able to go into the community unassisted. C1 have not returned to facility.

LPA obtained C1's admission agreement, dangerous propensities, supervision protocol for leaving the facility, and sign in/out sheet.

Exit interview conducted and a copy of this report was provided.
