On 11/7/2025, at 4:15 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct a case management visit to deliver an immediate exclusion letter. The LPA met with On Call Lead Direct Support Professional (DSP) Vanessa Flores-Martinez and informed her of the reason for the visit.
During visit, the LPA hand delivered the immediate exclusion letter for S1 to DSP Vanessa Flores-Martinez. The LPA was informed that S1 had not worked at the facility and was terminated before August 2025.
