Facility Evaluation Report
On 6/2/2026 at 11:20AM, Licensing Program Analyst (LPA) L. Hall conducted an unannounced annual required inspection. Licensee arrived at 11:45am and LPA explained the purpose of the visit. The administrator currently holds a certificate (#7023427735) that expires on 10/14/2026. Administrator, Ana Colunga, arrived at 1:10pm. The facility’s fire clearance was approved for four (4) ambulatory clients. No clients were present during the visit.
LPAs toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of four (4) bedrooms and two (2) bathrooms. All indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature for clients is maintained at 75 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 123.6 degrees F. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Hand washing poster, paper towel, and soap observed at all hand washing stations. The supply of extra hygiene was available for residents.
Smoke detectors/carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced 04/15/2026. Fire drill last conducted 03/30/2026. The emergency Disaster plan was last reviewed 01/05/2026. First aid kit was observed to be complete.
