Facility Evaluation Report
On this day, June 24, 2026, at 3:43 pm, Licensing Program Analyst (LPA) Delmundo arrived unannounced to conduct an annual required inspection. LPA was granted entry by staff, Edilburga 'Edie' Quillatupa Bernales, and informed the reason for visit. LPA also met with other staff, Luis Lopez Rodriquez. LPA spoke over the phone with Maria Christina Vazquez, administrator (ADM), who gave permission to the staff to be with LPA during inspection. ADM arrived at around 4:00 pm.
LPA started the inspection with the staff and continued with ADM. LPA inspected the kitchen, dining areas, living room, bedrooms, bathrooms, family room, front, side yard and backyard. Food supplies were observed good for 2 days of perishables and 7 days of non-perishables. Central storage for medications and kitchen drawer were sharps are kept were observed locked.
Fire extinguisher was observed fully charge with tag showed serviced June 29, 2026. Facility has carbon monoxide and smoke detectors that were tested and observed operational during today visit. Hot water temperature in one of common bathrooms was tested. Facility conducts shelter-in-place, fire and earthquake drills at least every quarter and records showed last conducted April 4, 2026.
