Facility Evaluation Report

On 4/30/2025 at 1:20pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 4/21/2025. LPA met with Nicolas, Nigel Grandez, Direct Support Professional. Administrator Jose Nunes, arrived at 1:30pm, and LPA explained the purpose of the visit.

The incident reported occurred on 1/19/2025. The incident report stated S1 and C1 was in the back yard. S1 came inside to charge C1's tablet and observed C2 having behaviors. C2's 1:1 (S2) requested assistance from S1. After S1 assisted S2 he went to the back yard and discovered C1 was missing. S1 then started to follow AWOL protocol for C1 and 9-1-1 was called. S3 stated S1 followed AWOL protocol. S3 also stated C1 haven't had this behavior of AWOL'g at the facility only at day program. S3 stated in May it's time for C1's ISP to be reviewed and it will include the AWOL'g behavior.

LPA reviewed and obtained the AWOL protocol, Individual Service Plan dated May 2024. (ISP) and the quarterly Individual Service Plan dated February 2025.
